Overview:

This is a nursing case management position for a case load of up to 60 IDD clients, including clients living in a group home setting. The LVN works with the RN, Host home providers, and direct care staff to ensure the health and safety of each client. The focus of this position is monitoring the client’s use of medications and monitoring health data and information and assisting in the health care aspects of the client’s daily needs. The successful candidate will be willing to travel in the D/FW area for face to face visits as needed, participate in annual planning conferences and be available to the families on the case load as needed. The successful candidate must be bilingual English/Spanish with an excellent working knowledge of medical terms.

Essential Duties:

  • Adhere to the guidelines set forth by the Board of Nursing (BON) and HCS Principles
  • Complete Annual Physical Assessments, as needed in a timely manner;
  • Administer medications;
    Monitors the consumer’s use of medications;
  • Monitor consumers’ health data and information;
    Assist the consumer in securing emergency medical services;
  • Make referrals for appropriate medical services;
  • Perform health care procedures ordered/prescribed by a physician/medical practitioner and required by standards of professional practice or law to be performed by licensed nursing personnel;
  • Monitor tasks delegated in accordance with state law to other service providers;
  • Complete tasks that cannot be delegated as determined by the RN;
  • Complete Medication Training for all Direct Care Staff, as needed;
  • Give first dose of new medication when medication delegation has been determined by the RN;
  • Participate in Annual and Special Planning Conferences;
  • Complete Quarterly Medication Reviews;
  • Complete monthly progress notes by the 3rd of the following month;
  • Turn in billing to the Billing Department by the 3rd of each month;
  • Order medication, as needed for the Group Homes;
  • Make sure a 72-hour supply of all medications are on hand at the Group Homes;
  • Agree to be on call after office hours, as needed to assist the consumers;
  • Respond appropriately to all consumer crises;
  • Participate in scheduled department and all staff meetings;
  • Keep assisted consumer charts survey ready at all times;
  • Keep license current and in good standing.
